Excercise Limitations with a PVD?

Frome the comments section of my video on Posterior Vitreous Detachments:


"How soon can I get back to my carpentry? Doctor told me to stay away from heavy lifting for 1 week. I started to experience flashes on left peripheral Sunday evening and I saw Ophthalmologist Tuesday and she confirmed PVD but she need not see me again unless further developments"

IF I were able to ask your doctor the reasoning behind their recommendation to avoid lifting, I would ask "What it is about lifting heavy things that would put you at risk?". There is admittedly a 'window' of time where you are at greater risk of peripheral retinal holes, tears, and detachments and that is while the vitreous is detaching.. the process of the PVD. A PVD can complete quickly or it may take weeks or months for the outer vitreous cortex membrane to fully separate. Here is an example: If you use painter's tape to tape an edge next to the door trim and later after the paint dries, there is zero chance the paint on the wall will just spontaneously peel away. It is while you are peeling the tape away from the wall - that is the riskiest time. If you can peel away the tape without affecting the underlying paint then 'success' and now there is again a zero chance of paint peeling off. If you are experiencing flashes of light, that is strong evidence that the vitreous membrane is still attached and pulling and tugging.

Now back to anatomy and physics: When you lift a weight, there is of course the muscular strain, but there can also be an increase in intrathoracic pressure which can then increase venous pressure. None of these would affect the vitreous, which is essentially 100% fluid and fluids are incompressible. Furthermore, lift a weight would not create turbulence or movement of the vitreous more than just simple L > R > L movements of the eye. I am not your doctor, and I am not trying to undermine your doctor-patient relationship and trust, but I do not think there should be any particular restriction of activities unless there is an actual diagnosis of a retinal hole or tear or detachment.
